Thank you! Both problems are now resolved.
I do have one last question: I am looking to be able to quickly search key words within a tracker summary or description without having to set up advanced queries. I was hoping the search box on the top right header would do this but it keeps on returning no result found.
For example I have four trackers with sentences in the summary and description that contain the word "test". When I search for the word test I get no result.

Headermenu database code has been updated.
you need to run upgrade-db.php script available in src/db/
php -f upgrade-db.php headermenu
